Only the largest, Blue Cross Blue Shield of Massachusetts, eked out operat - ing income, finishing the year with $7.6 million, following two years of operating losses. Fallon Health, Tufts Health Plan and Harvard Pilgrim Health Care each saw operating losses mount, due to increased enrollment and utiliza - tion under the Affordable Care Act.
