As a result, CHC has become a leading independent, non-profit healthcare provider in the state of Connecticut, providing comprehensive primary care services in medicine, dentistry, and behavioral health to more than 145,000 people. Committed to the underserved… Our programs are available to all, but we have a special commitment to the uninsured, underinsured, and key populations such as patients with HIV/AIDS. For a constantly growing number of people in practically every Connecticut town, we are the healthcare organization that works to keep our patients — and our communities — healthy. … and committed to going beyond traditional care CHC practices what is widely considered the future course of healthcare delivery. We incorporate prevention and health promotion, treatment of illness, and management of chronic disease - oen all during the same visit. Using innovative service delivery models and state of the art technology, CHC goes beyond traditional health services to bring care to the places it is most needed, and most impactful. ABOUT US W e opened our first office in 1972, a converted second-floor apart- ment in Middletown that we ran as a free clinic. At that time, access to health care was considered a privilege. We were a group of local activists and Wesleyan University students, concerned about the cost and lack of access for our friends and neighbors in the community. Starting with dental care, we began to build a system of primary care. From that one location serving a hand- ful of people, to today in over 200 locations throughout Connecticut, we care for 145,000 patients, providing pri- mary medical, dental and behavioral health services. Along that journey we realized that if you're going to provide pri- mary care, you have an obligation to improve it. at led us to form the Weitzman Institute, doing research on better ways to deliver care to those in need. Today, Weitzman Institute initiatives are operating in health systems from Hawaii to Rhode Island — workforce training, tele-health, and complex care management to name a few. May 1, 2017 marks our 45th anniversary. During that time, CHC has held steadfast to values of social justice and our founding vision that health care is a right, not a privilege. is past year, we opened a new site in Hartford in response to a request from Connecticut Children's Medical Center, asking us to take over one of the largest pediatric prac- tices in New England. We responded as we have in the past —with excitement. is value of responsiveness extends to our employees and our com- mitment to their success. We take pride in having established a mini- mum wage of $18 per hour. Coupled with our tuition reimbursement and workforce-development programs, we are a force for economic growth and opportunity in our communities.
