NEW HAMPSHIRE: First in the Nation • Profiles in Excellence 171 Harvard Pilgrim also teamed up with Foundation Medi- cal Partners, the physician group of Southern New Hamp- shire Health System, on a patient-centered medical home pilot. is care model provides a more patient-centered experience by emphasizing greater care coordination, team- work and patient involvement. Process changes enacted during the pilot program resulted in increased patient im- munization rates and fewer emergency room visits. Together with Bedford Ambulatory Surgery Center, Harvard Pilgrim utilized a bundled payment arrangement for screening colonoscopies. Instead of paying a sepa- rate fee for each individual component of the procedure – including charges for physicians' services, anesthesia, pathology, etc. – Harvard Pilgrim paid Bedford Ambula- tory Surgery Center a fixed lump sum for each patient's care. Such arrangements focus on using all of the required resources appropriately and help make cost and quality more transparent for patients. Community investments For more than 30 years, the Harvard Pilgrim Health Care Foundation has provided the tools, training, volun- teers and leadership to help build healthy communities. Since its inception, the Foundation has awarded more than $133 million to support nonprofit organizations, enhance children's well-being and make health care more equitable. In 2014, through its Mini-Grant program, the Foundation distributed nearly $556,000 in employee- directed Mini-Grants to more than 650 nonprofit organizations throughout the region. In 2014, Harvard Pilgrim employees worked more than 3,200 volunteer hours to serve nonprofit causes. Locally, we have partnered with City Year New Hamp- shire for annual community service days at Beech Street School in Manchester. Volunteers have helped improve the school environment by painting murals, building bookshelves for the library and classrooms, and more. e Foundation has also invested in New Hampshire programs that help enhance children's well-being, such as CATCH Kids Club and NH Let's Go! Healthcare. In 2014, the Foundation awarded $75,000 to FEEDNH.org, which sent local chefs from Great New Hampshire Restaurants to teach healthy cooking classes at six Boys & Girls Clubs throughout the state. The Foundation's work to help children eat better and move more has created an opportunity to help families and communities be healthier – starting with what they eat. In 2015, the Foundation is launching the Harvard Pilgrim Healthy Food Fund to support nonprofit organizations across the region that make fresh – and often local – food easier to find and buy.
