have received natural gas service. Applicant must execute a customer agreement with Summit by 9/30/2017 and must complete conversion to natural gas and begin receiving natural gas service
by 12/31/2017. The amount of the rebate available to Applicant is subject to the terms and conditions in Summit's tariff and, in certain instances, pre-approval by Summit. Applicant must submit
verifi able documentation of conversion. The rebate may only be combined with Summit rebates and incentives provided for in Summit's tariff , and cannot be combined with other Summit promotions.
