Notes: Number of hours worked by licensed staff (RNs, LPNs/LVNs) each day per resident is computed by adding the number of RN and LPN/LVN staff hours. A health deficiency is a finding that a nursing home failed to meet one or more federal health requirements during an annual health inspection or a complaint inspection. Inspectors identify health deficiencies by observing the nursing home's performance, its practices or the conditions in the facility. An example would be not having consistent infection-control practices. The average number of health deficiencies in Connecticut is 7.7 per skilled-nursing facility, according to Medicare.gov. Penalties may be imposed on a nursing home when a serious deficiency is cited, or if the nursing home fails to correct a deficiency for a long period of time. Penalties consist of fines and payment denials. For this list, nursing homes only received fines. Fines may be imposed once per deficiency or each day until the nursing home corrects the deficiency. Wethersfield Health Center is now closed. (1) Formerly Brittany Farms Health Center. (2) National Health Care Associates (NHCA), parent of Hebrew Home for Health and Rehabilitation LLC, took over operations in Aug. 2016 upon Hebrew HealthCare's chapter 11 bankruptcy filing. (3) Formerly Aurora Senior Living Of Cromwell. (4) Formerly Kettle Brook Care Center LLC. —Compiled by Brendan Sullivan.
