Health Care Resource Guide • 2016 – 2017 5 W e are happy to once again support the WBJ's Central Mass/ Metrowest Health Care Resource Guide. e last year has been a turning point for our organization. In April, the divisions that create our full complement of home care, palliative care, hospice, and wellness services, VNA Care Network, VNA of Boston, VNA Hospice & Palliative Care, VNA Care Advantage, and VNA First choice, came together under the new parent name VNA Care. For more than 130 years, the divisions of VNA Care have served the home health and hospice needs of millions of patients throughout Eastern and Central Massachusetts. VNA Care continues that great legacy with the same mission, the same dedicated staff, and the same exceptional care you have come to know. e new look and name more accurately represents the services available to patients and the com- munity and we are confident it will help to facilitate and coordinate referrals with our partner health care providers. VNA Care has redefined the way care is delivered in the home, in- corporating technology and advanced clini- cal care practice to meet the needs of today's home care patient. Home care clinicians are able to care for a new mom bringing home a baby that was born prematurely, a 30-year- old recovering from knee surgery who opted to avoid a stint in a rehab facility, and an el- derly patient in an assisted living community managing multiple medications. rough our three hospice homes, coupled with our ability to provide palliative and hospice care wherever the patient lives, our staff supports those facing the challenges of a terminal ill- ness. ese are just a few of the scenarios VNA Care faces each day. ank you for allowing us to be a part of keeping the heart of the Commonwealth healthy for more than 130 years! Bringing Health Care Home for 130 Years T he new logo Reliant Medical Group introduced last spring is a col- orful representation of a healthcare organization that has distin- guished itself for decades in Central Massachusetts – and through- out the country – for quality and innovation. ree interlocking arcs in vivid blue and green hues remind us of the essential relationship among caregiver, patient, and community. Beyond symbolism, how- ever, our new logo performs another important job. For reasons that made a lot of sense in the past, Reliant Medical Group (formerly Fallon Clinic) established healthcare subsidiaries with different names and logos. e result of having multiple brand names was that patients and others may not have readily recognized the impressive scope and breadth of our medical group. Did people know, for example, that Reliant had a modern, free- standing endoscopy center on Plantation Street in Worcester? Were they aware that the company delivering medical equipment to their homes may have been operated by Reliant? And did they know that Southboro Medical Group and Reliant Medical Group became one organization in 2015? Chances are, the answer was no. But all this has changed now that the Reli- ant family of services is unified through the use of one distinctive new logo. Today, we look like what we are: A dynamic healthcare organization with a wide array of medical services for people of all ages. anks to a bold new look, our patients – and future patients – can better understand all that Reliant Medical Group offers to the people of Central and Met- ro West Massachusetts.
