Subject to terms and conditions set forth in Summit's tariff . The program is for a limited time only. Applicant must qualify as a commercial customer and cannot previously
have been converted to natural gas or otherwise received natural gas service. The program consists of two separate rebates ("Primary and Supplemental"), and to be
eligible for any rebate in excess of $1,500, Applicant must complete Summit's Commercial Rebate Request Forms for both rebates. Applicant must execute a commercial
customer agreement with Summit for natural gas service by 08/31/2016 and must complete the natural gas conversion and begin receiving natural gas from Summit by
12/31/2016. Applicant must submit verifi able documentation of the conversion. The amount of the rebates for which Applicant may qualify is subject to terms and conditions
and, in certain instances, is subject to pre-approval by Summit. The rebates may only be combined with Summit rebates and incentives provided for in Summit's tariff , and
