Designated as a Federally Qualified Health Center Look-Alike (FQHC LA), InterCommunity, provides same-day primary care services, including but not limited to: complete physical exams, blood work, vaccinations, lab work, sick visits, medication management, nutrition counseling and patient education and materials. InterCommunity's Behavioral Health division is also located at 281 Main Street in East Hartford, CT. Services include individual and group therapy for children and adults, medication management, case management, employment, supports, as well as Young Adult and Latino services. InterCommunity's Addiction Services division offers a similarly wide range of substance use treatment and recovery-supportive services, including residential and outpatient treatment, with many sites in the Hartford area. GOALS • Provide services that will benefit whole person health through our primary care, behavioral health and addiction service divisions. • Continue to build and enhance our primary care services through our Federally Qualified Health Center Look-Alike designation. • Continue to fully integrate addiction services into the InterCommunity spectrum of care • Continue to provide same day access, so that patients are seen the same day that they call or walk in our doors. • Continue to provide high quality, superior services that are available regardless of insurance, phase of life, acuity of issue, or town of residence • Expand and grow in location throughout East Hartford and Hartford in order to fully service the population. • Continue to deliver an emphasis on employee well-being, self-care and appreciation FUNDRAISING EVENTS/OPPORTUNITIES Every year InterCommunity hosts its InterActions to help generate funds to continue providing Whole Person Health Care. InterCommunity's main Signature event will be held Thursday, November 5, 2015 at Real Art Ways in Hartford, CT.
