Also, Healthy Futures, Har- vard Pilgrim's groundbreaking incentive-based program for improving employee wellness, rewards members for taking steps to improve their health. Throughout Maine, Massachusetts and New Hampshire, Harvard Pilgrim's extensive network includes doctors and hospitals that members know and trust. When employees live and work in different parts of the country, employers can turn to Harvard Pilgrim for national coverage solutions through an al- liance with UnitedHealthcare. Harvard Pilgrim has long been recognized for its leadership and innovation in prevention and disease management. Whether it's through online tools or work- ing directly with a personal health coach, Harvard Pil- grim provides the necessary resources to help members make healthier lifestyle decisions. Harvard Pilgrim's care management programs and dedicated nurse care manag- ers also help members deal with conditions such as asth- ma, diabetes, coronary artery disease or other complex conditions to help minimize complications and improve their well-being. Through Harvard Pilgrim's Quality Grants pro- gram, physician practices in Maine, Massachusetts and New Hampshire received close to $1 million in 2011 to fund initiatives that support the patient-centered medi- cal home concept. This emerging care delivery model encourages partnerships between patients and primary care physicians to help achieve better health outcomes. In the patient-centered medical home approach, care teams attend to the multifaceted needs of patients and provide "whole person," comprehensive and coordi- nated patient-centered care. The needs and desires of pa- tients come first in an environment where patients have collaborative relationships with their primary care physi- cians and care team. In Maine, four provider groups — Quality Counts in Augusta; St. Mary's Regional Medical Center in Lewis- ton; Maine Medical Center PHO in Portland; and Nova- Health, LLC in Yarmouth — have received grants from Harvard Pilgrim for initiatives that focus on one or more Harvard Pilgrim is Maine's only local, not-for-profit health plan, and it is recognized across the country for outstanding quality and customer service.
