Patients are also putting personal technology tools to good use when it comes to health. Smartphone apps enable them to monitor heart rates, blood sugar and oxygen levels, body mass index and other metrics. Com- fortable with at-home self-care, some patients are now using medical de- vices such as pulse oximeters, spirom- eters, blood pressure cuffs and other monitoring devices. at data can then be instantly transmitted to pro- viders who can monitor health condi- tions remotely, avoiding unnecessary hospitalizations and reducing health care costs. Also, the growing popularity of ge- nomics is helping identify personal health risks and determine specific screenings and interventions to main- tain good health. Personalized medi- cine, based on genomic test results, will treat the individual rather than follow a one-size-fits-all approach. Hospitals are contributing to the pub- lic discourse by studying population health analytics. As hospitals collect data and store it in centralized data warehouses, the medical community and the public can use the informa- tion to help determine appropriate treatment options. Growing attention to patient-driv- en care prompted the establishment of the patient centered medical home (PCMH), a model that organizes and coordinates patient care across the health spectrum with an emphasis on accessibility, quality, safety and keep- ing the person healthy throughout his lifetime. Also, accountable care orga- nizations (ACO), a variety of provid- ers that includes internists, cardiolo- gists and orthopedists to neurologists, gastroenterologists, mental health professionals and others assembled within one organization, deliver coor- dinated care that will eliminate errors and duplication of effort while reduc- ing costs. Consumers are now lobbying for price transparency and providers, hospitals and insurers are respond- ing. For example, some medical prac- tices and hospitals list the costs of office visits and routine procedures on their websites.
