Coordinated care in action: Janet's Story For Janet, an Anthem member in Connecticut living with breast cancer that had metastasized, the next step in treatment presented an unexpected challenge. Her care team recommended proton beam radiation therapy, but the service was not available in Connecticut. e only option was treatment at an out-of-network facility in another state, which required special approval for coverage. Understanding both the urgency of treatment and the emotional toll of the situation, an Anthem nurse case manager stepped in to help. She conducted extensive research, worked directly with the facility, and successfully negotiated a significant discount on the proposed charges. e case manager then coordinated the approval process to secure the necessary agreement and prevent delays in care. roughout the experience, she provided ongoing communication, advocacy, and reassurance, helping Janet focus on her treatment rather than administrative hurdles. Michael's Story Another Connecticut member, Michael, faced a different challenge after suffering a stroke that affected his mobility. Following his hospitalization and rehabilitation stay, an Anthem nurse case manager collaborated with the discharge team and Michael's family to ensure he had the support needed for a safe transition home. e case manager identified critical gaps during the recovery process, including a missing blood pressure medication regimen and uncertainty surrounding prescription transfers after the closure of the family's local pharmacy. By coordinating with pharmacists and healthcare providers, she helped secure timely access to medications and arranged follow-up care. She also worked closely with Michael's family to facilitate outpatient rehabilitation services and provide education on fall prevention and safety measures. rough proactive outreach and coordination, Michael received the support necessary to continue his recovery with confidence. While their health journeys were very different, both stories highlight the power of care coordination and personalized support. In each case, dedicated nurse case managers served as advocates, coordinators, and trusted partners, helping members overcome barriers, access appropriate care, and navigate complex healthcare systems during some of life's most challenging moments.
