Seven Hills Foundation and Affiliates connect and integrate the needs of people with brain injury, disabilities,
and autism; seniors; vulnerable children and families; people living with mental illness; and more. In Greater
Worcester, there is a growing demand for combined health and social care. Seven Hills' response is evolving with
innovative strategies, such as:
n People with disabilities pursue meaningful community-based programs and employment.
n
Grief support is available at no cost to children and families who have lost a loved one.
n Food insecurity & basic needs of families are supported through Family Resource Centers.
n Senior care options include Home Care, Adult Family Care, and the Senior Companion program.
n At-risk youth access mentoring, academic, and social support, and a path to college.
n Behavioral Health care features centralized referral and medical collaboration.
Seven Hills is the Catalyst for Care in the Community, transforming the lives of people who are benefiting from
