The dedication, compassion, and sacrifices you, and your loved ones, are making is inspiring. You are true heroes, thank you! Likewise, we thank everyone in the community for their support, donations, and commitment to social distancing. Together we will come through these challenging times and emerge wiser and more robust. SOHO is committed to doing everything possible to safely shepherd our providers and patients through these unprec- edented challenges to ensure our healthcare system and clinical networks remain stable and sustainable. To that end, SOHO has initiatied the following: Healthcare Providers: SOHO is developing and curating communication tools and resources for providers to navigate our network through the morass of information related to the COVID19 pandemic, including: • Creation of the COVID-19 Information & Resources web- site with easy access to testing protocols, financial assistance resources, patient-facing materials, payor policies, telehealth resources, clinical algorithms, and more. • SOHO COVID-19 Command Center—a weekly webcast forum for sharing new information with our providers and offering the opportunity to collaborate, ask questions, and share concerns. • Informed advocacy to raise awarenss and support for our healthcare providers and system. SOHO is advocating at state and federal levels for financial assistance for hospitals and medical practices, addressing medical supply shortages, seeking protection for value-based healthcare arrangements and more. Letters have been sent to the Secretary of the US Treasury, US Small Business Administration, Senator Chris- topher Murphy, and Governor Ned Lamont calling for aid to mitigate the severe financial implications COVID-19 is having on our healthcare system. Patients: Patients are receiving high-quality active care manage- ment from the SOHO Population Health Team — comprised of nurses, health coaches, pharmacists and more. The team works daily to engage and assist our patients with chronic conditions and COVID signs and symptoms to ensure timely access to care and the mitigation of social isolation. Post-COVID, SOHO will be a healthy, viable network of hos- pitals and providers that will continue to focus on innovation and value because the actions we have taken amidst this cha- os will support our future efforts. Our network of committed hospitals and providers will set the stage for redefining our state and nation's healthcare system for years to come. We will continue to serve our communities by enhancing our partnerships with key stakeholders to achieve these goals.
