Who We Are Independent and non-profit, Connecticut Community Care is Connecticut's leading care management organization and advocate. We are nurses, social workers, counselors and experienced administrators, familiar with clinical, legal and social aspects of providing community care. What We Do Partnering with individuals and families, we serve as the liaison between you and the wide, often overwhelming, world of care providers, resources and regulators. We help make sure nothing stands in the way of getting the right care, at the right time in the right place, home in your own community. GOALS • Capitalize on our commitment to provide quality- based care management service to our clients as evidenced by our 40-year history in the business and our recent achievement of the highest status of Case Management Accreditation for Long Term Services and Supports (LTSS) from the National Committee for Quality Assurance (NCQA). • Attract capable, caring, competent employees to help achieve our mission. • Focus on technology upgrades that provide our care management team with state-of-the-art systems and software for simpler access to client information and tracking of important data leading to improved outcomes for everyone. "Interoperability" with other entities in the health and community care arena is essential to improved quality. • Engage the public and philanthropic organizations in helping those we serve by improving their quality of life at home and in their communities with enhanced donor funding and support. • Partner with local and state government, business leaders and community organizations to integrate health and wellness programs into community settings. • Support communities in their efforts to respond to their aging population.
